Your paid-leave claim was denied. Read the reason before rewriting everything

An appeal works best when it answers the actual issue in the decision.

A denial letter can make the whole plan feel like it has collapsed. Before assuming you do not qualify, identify exactly what was denied and why. A missing document, a wage-record error, an eligibility finding, and a disputed medical period call for different responses.

Save the complete decision, including attachments and the date it was sent. The instructions on that notice matter more than a generic appeal template found online.

Find the deadline first

Write down the appeal or review deadline, where the response must go, and what counts as timely submission. Different programs use different procedures. Some offer reconsideration; some use a formal appeal or arbitration process. Do not assume a phone conversation pauses the deadline.

If the instructions are unclear, contact the program promptly and document the answer. Ask whether additional evidence can follow a timely appeal if you are still waiting for a provider or employer record.

Match evidence to the stated reason

If wages are missing, gather the relevant pay records and identify the employer and period. If relationship evidence is missing, provide the requested proof of birth or placement. If dates conflict, explain the correct sequence with supporting documents.

For a medical determination, ask the treating provider to address the actual certification question. More pages are not automatically stronger evidence. A clear answer about the disputed period or work limitation may be more useful than unrelated records.

Write a short factual response

Use the claim number, decision date, disputed finding, correction requested, and a numbered list of attachments. Keep a copy of everything submitted and proof of receipt.

I am requesting review of the decision dated __ for claim __. The decision states __. I believe this is incorrect because __. Attached are __. Please confirm receipt and advise whether further information is required.

That is a structure, not legal advice or a guarantee of success. Complex eligibility disputes may need help from the program, a legal-aid organization, or an attorney.

Keep the employer informed separately

A benefit appeal does not automatically extend approved time away or job protection. Ask HR what happens while the appeal is pending and whether any dates need a separate request. Update the budget using a pending-payment assumption, not a promised award.

Finally, keep the appeal organized around the issue the decision identifies. You do not have to prove that parenting is hard or that you deserve time with your child. You need the reviewer to see the relevant rule, the relevant facts, and the specific correction you are asking them to make.
